Most accommodation letters get written from scratch every time, even though 80% of letter content is repeated across students. This template pack gives DRC coordinators 12 pre-written, FERPA-compliant accommodation letter templates organized by accommodation type — covering extended testing time, note-taking, alternative formats, mental health accommodations, mobility, attendance flexibility, and more.
Usage guide: These templates are starting points. Customize bracketed fields ([Student Name], [end date], etc.) and adjust language to match your institution's tone and policies. Always include your DRC contact information, FERPA notice, and any institution-specific procedures. AccommodoHub generates these letters automatically with student-specific details pulled from the accommodation record.
[Student Name] is a registered student with the Disability Resource Center and is eligible for extended testing time of 1.5x the standard time on all exams, quizzes, and timed assessments in your course. This accommodation is to be implemented for all timed assessments throughout the semester. The student will coordinate with you and/or the Testing Center to schedule extended-time exams. Please contact the DRC if you have questions about implementation.
[Student Name] is registered with the Disability Resource Center and is approved for extended testing time of 2x the standard time on all exams and timed assessments. Given the extent of this accommodation, the student typically takes exams at the institution's Testing Center, which has scheduling and proctoring infrastructure for extended-time exams.
[Student Name] is approved for a distraction-reduced testing environment for all exams and quizzes. This typically means a separate, quiet room or the institution's Testing Center. Please coordinate with the student regarding exam logistics, or refer them to the Testing Center for direct scheduling.
[Student Name] is approved for note-taking services in your course. The DRC will coordinate with a peer note-taker (or AI-assisted note capture, depending on student preference). Please support this accommodation by allowing recording devices in class if requested, and by sharing presentation materials with the student in advance.
[Student Name] is approved to receive course materials in an alternative format ([Braille / large print / electronic text / audio]). The DRC's Alternative Materials team will coordinate with you to convert required course materials. Please share your reading list, syllabus, and any in-class handouts with the DRC at the start of the semester.
[Student Name] has a chronic condition that may necessitate occasional absence from class. The student is approved for attendance flexibility, meaning that disability-related absences should not negatively impact their grade. This accommodation does not waive course requirements; the student remains responsible for all course content and assessments. Please work with the student to establish a plan for missed sessions.
[Student Name] is approved to record course lectures as an accommodation. Recordings are for the student's personal academic use only and may not be shared or distributed. Please permit the student to record class sessions; the student has signed an acknowledgment regarding appropriate use.
[Student Name] is approved to use assistive technology in class and during assessments, including [screen reader / FM system / captioning / speech-to-text software / smartpen]. Please permit the use of this technology in your course. The DRC has verified that the technology is appropriate and will not provide unfair advantage.
[Student Name] is approved to receive [sign language interpreting / CART (Communication Access Realtime Translation)] services in your course. The DRC's Deaf/HoH Services team coordinates interpreter or CART scheduling. Please provide course materials, recorded lectures, and any video content with captions to the DRC for advance preparation.
[Student Name] is registered with the DRC for a mental health condition. Approved accommodations include [extended testing time / attendance flexibility / breaks during exams / alternative testing environment / extended deadlines on assignments by 24-48 hours with prior notification]. Please coordinate with the student regarding implementation. The student's specific diagnosis is not disclosed; only the approved accommodations are shared with faculty.
[Student Name] has a mobility-related disability and may require [accessible classroom / accessible seating / accessible lab equipment / accessible field trip arrangements]. Please ensure that course activities, including any off-site components, are physically accessible. Contact the DRC if alternative arrangements are needed for any course component.
[Student Name] is currently approved for temporary accommodations through [end date] due to a temporary medical condition. Approved temporary accommodations include [extended testing time / extended deadlines / attendance flexibility / note-taking support / use of assistive technology]. These accommodations expire on the date listed and will be reviewed by the DRC if the condition persists.
